Applicants submitting to the FY2011 Ohio Third Frontier Medical Imaging Program should be aware of the additional requirement to comply with Executive Order 2010-09S. The Executive Order and the required forms can be found in Attachment EO on the Medical Imaging Program 2011 page. Applicants are required to submit the completed Affirmation and Disclosure Form with their proposal.
Governor Ted Strickland issued Executive Order 2010-09S "Banning the Expenditures of Public Funds for Offshore Services" on August 6, 2010. The Executive Order prohibits the use of any funds within the control of an executive agency to purchase services which will be provided outside of the United States. The Executive Order became effective on August 6, 2010, requiring all agency compliance beginning on that date.
The Executive Order applies to grants made by the Ohio Development Services Agency. Grantees may not use any grant funds to pay or be reimbursed for services provided offshore to implement the proposed project or program. In addition, grantees may not count as match or cost share specifically required as a condition of the grant the costs paid for any offshore services.
To be considered by the Ohio Development Services Agency, a proposal or application must be accompanied by an Affirmation and Disclosure in the form attached to this RFP or application. The Affirmation and Disclosure must be signed at the end by an authorized representative of the proposer.

How does Development wish to receive Executive Order 2010-09S Affirmation and Disclosure Form to be accompanied as part of the proposal package? Is it acceptable to fill in the 3-page Affirmation and Disclosure Form by hand? Is it acceptable that only one Affirmation and Disclosure Form accompany the entire proposal package? By entire proposal package, this means the original paper version marked "original", the three additional paper copies marked "copy", and the two CDs containing unlocked .PDFs. Does Development want the Affirmation and Disclosure Form somehow attached to every single "copy" and "original" version of the proposal? If this is in fact the case, where (in what order) should this form be included per proposal?
It is acceptable to complete by hand the Affirmation and Disclosure Form since it is provided as a .pdf rather than a Word document. The original signed Affirmation and Disclosure Form and three hard copies should be included with an applicant's proposal submission. If an applicant has scanner capabilities, a signed .pdf version of the form should be included on both CDs. The original signed Affirmation and Disclosure Form should be attached as part of the stapled original version of the proposal behind the collaborator letters. A copy of the signed Affirmation and Disclosure Form should be attached as part of each of the three stapled copy versions of the proposal behind the collaborator letters.
